Home Based Rehabilitation

Consumers who are medically stable and can benefit from a comprehensive rehabilitation program can now receive services within their home and community setting.

Services may include speech, physical and occupational therapy, a one-on-one residential life skills person throughout the day, psychiatric and physician consultation, cognitive remediation, school re-entry and vocational training.

This program boasts success in school re-entry, family satisfaction, convenience and flexibility for all involved.

Services offered include:

  1. Implementing treatment plan formulated by assessment team and/or transfer team.
  2. Offering structured opportunities and training in the following areas: personal care management; personal health management and education; self-care/activities of daily living; advocacy; transportation; disability adjustment; recreation/leisure; physical endurance; and community integration.
  3. Assisting in enhancement and development of social/leisure support networks.
  4. Teaching and promoting the use of strategies for use in functional living and working environment.
  5. Use of natural consequences and the opportunity for choice and acceptable options to minimize reliance on others and maximize independence.
  6. Access to clinical services which may include:
  • occupational therapy
  • physical therapy
  • speech language therapy
  • neuropsychology
  • psychology
  • psychiatric
  • vocational
  • school re-entry

7. One-on-one residential life skills person throughout the day.
8. Case Management to coordinate and oversee psychological /medical intervention.
